Please make sure your child returns the following items by Friday, August 26, 2016. It is necessary to have one emergency card for each child. Remember to update these cards throughout the school year when changes occur. We must be able to contact someone on the card in case of an emergency. If a person is not on the card, he/she will not be allowed to sign out your child. Current telephone numbers are very important. All students who return these items by Friday, August 26, 2016 are eligible for our Friday Good Guy Recess:

_____ Over the Counter Product Authorization- Print and sign form.
_____ Parent-Student Handbook (review with students)- Review handbook with your child.
_____ Insurance Verification form- Print and sign cover letter, brochures will be sent home on the first day of school.
_____ Native American Education Program (optional)- Print and sign form if applicable.

- OUSD Parent Portal Account Request- Parents can view grades and missing assignments through Parent Portal in grades 4-6 only. This form is only needed to request a new account, or to report problems with an existing account. This form must be returned to the school office in person. You must provide a valid picture ID when you return this form.
